Your First Visit

At Midtown Smiles Dental Care, new patients are always welcome.

Your first visit to our office will primarily be for the purpose of gathering information necessary for us to plan your future dental care.

This will include:

1. Past dental history
2. Medical history
3. Schedules of any medication being taken
4. X-Rays (current and any past x-rays forwarded to our office)

We will perform an extensive and comprehensive examination of the teeth, gums, bite, joints, and soft tissue. When needed, and with your permission, we will utilize all appropriate x-rays, as well as intra-oral photographs of your teeth and jaws to identify areas of concern. We believe this in-depth examination to be the cornerstone of a successful dentist-patient relationship.

This is the best time for you to communicate to us any concerns, fears, and expectations which you may have regarding your dental health. Based on the above findings, Dr. Shetty with your input will establish a diagnosis and a treatment plan that is customized to your needs.
Please arrive 15 minutes prior to your appointment and bring the completed new patient forms below with you. Also, please bring a list of any current medications you are taking and any dental insurance information. Parents and/or guardians must accompany all patients under the age of 18.
